REGISTRATION & FEES

The O'Fallon Kixx program is open to both residents and non-residents of O’Fallon. Try-outs are mandatory for all teams U9 and older. If your child has missed open tryouts, please contact Keith Richter at (618) 624-0139 extension 5103 to find out about any openings on existing teams. A try-out can then be scheduled for your child if there are openings available.

The following forms must be filled out and collected at registration:

IYSA Medical Release Form

O’Fallon Kixx Permission To Roster Form

Copies of the Following Documents Are Required To Complete Registration:

1. Copy of the player’s birth certificate (NOT the hospital version, this MUST BE the local government copy)

2. Front and back copy of the player’s insurance card

3. A small picture of the player or digital photo to place on the player’s IYSA player card will be required after registration is completed.

Payment of Fees

Payment of fees is handled by the O’Fallon Parks and Recreation Office. The fees can be mailed in or paid in person. Checks are to be made out to “O’Fallon Parks and Recreation”. There is a payment plan available. Please see details below:

Payment Plan:

U9 – U17 SISL: $400.00 Paid in full with cash, check or credit card. Option is $100.00 credit card payment Aug 1st, with an automatic deduct of $100.00 Sep 1, Oct 1 and Nov 1.

U9 – U17 SLYSA: $600.00 Paid in full with cash, check or credit card. Option is $150.00 credit card payment Aug 1st, with an automatic deduct of $150.00 Sep 1, Oct 1 and Nov 1.

Uniform Information:

Uniforms will be purchased through C & C Sports Stop. The address for C & C Sports Stop is listed below. Uniforms orders should be placed as a team and coordinated through the team manager or a team parent!  C & C Sports Stop also offers O’Fallon Kixx Spirit Wear.
